The whole concept of the horror genre is to generate fear. This is most commonly done using jump scares, a surprise attack which is usually accompanied with a very loud sound effect.

But what truly separates a good horror game or movie from a great one is the actual build up. People sometimes use the phrase "the suspense is killing me" and in the horror industry this is taken literally.

Fear is what we naturally feel when we're not in full control of the situation. It's when some or all of our senses are confused. Limited sight like a dark room would not only limit your perception, it would also raise your awareness of sounds and smell. If you then started hearing movement in a dark room when you should be alone, you'll start to feel true fear. The human imagination can thus also be deadly. Even before you've actually seen the threat, your mind will imagine the worse possible scenario causing unnecessary fear. The longer it takes until the confrontation, the stronger the fear becomes. Fearing a certain situation can even cause you to feel uncomfortable weeks in advance. You might feel like you want to throw up or it might even cause anxiety. You'll start avoiding usually safe areas and turning down invitations just to avoid confronting your fear. Some people even avoid medical treatments and checkups because they are terrified of needles.

Fear can even stimulate your kidney's urine production and the adrenaline rush caused by your fear might make you want to relieve yourself. Other times it's the exact opposite as some people can't urinate due to the fear.
